New clients can set up their Patient Portal account with the help of our Access Team.
Current clients who would like to use the Patient Portal account should reach out to their provider’s office for assistance.

- Call our main number at (812) 537-1302
- Select Option #6 from the phone menu
- Leave a message with the following:
- Full Name
- Date of Birth
- Cell Phone and/or Email Address (for username/password resets via text or email)
- A brief description of the issue (forgot username or password, can’t log in, etc.)
