With the portal, you can stay connected to your care, review important information, and take an active role in your wellness journey.

Registration
Same Day Access clients will set up their Patient Portal account with the help of our Access Team.
We do not offer the Patient Portal for current clients at this time. Please reach out to your provider for a copy of your safety plan or treatment plan if needed.

If you are having trouble accessing your Patient Portal, you can reach out via phone or through our convenient contact form. Please allow 1 business day to respond to your request.

- Call our main number at (812) 537-1302
- Select Option #6 from the phone menu
- Leave a message with the following:
- Full Name
- Date of Birth
- Cell Phone and/or Email Address (for username/password resets via text or email)
- A brief description of the issue (forgot username or password, can’t log in, etc.)
